• Share your health-related goals, enabling us to better understand and support your needs.
• List any chronic medical conditions you may have, helping us tailor our care to you.
• Detail all the medications and supplements you regularly take, including dosage and frequency. (Before you begin, it would be helpful to gather all those pills from the medicines cupboard, kitchen counter, and fridge onto one tray. Don’t forget your reading glasses for those fine print labels).
• Record your height and baseline weight, enabling us to track your progress. (Remember to measure yourself before starting the form).
